One Medical Passport is a secure, online portal that allows patients to Pre-Register from the comfort of their own home in as little as 20-25 minutes. Patients create a password protected account to complete a questionnaire regarding their medical history. Once completed, a Pre-Operative nurse will review it and contact you closer to the date of your scheduled visit.
